Investigation Reports VA Hospital Fatal Incident Was Caused By 'Pressure Event'

An investigation into the fatal incident last Friday at the Veterans Affairs medical campus in West Haven concludes that there was no fire or explosion. Instead the report describes it as a “pressure event” within the steam system. The event caused the space to fill up with heated vapor air killing two people and injuring three others. The two men killed were Euel Sims, a plumbing executive at the hospital and Joseph O’Donnell, an outside contractor.
